Abstract
Introduction:
Myopathy is the most common side effect of statins. Since nitric oxide (NO) has a key role in regulating skeletal muscle function, we studied whether the NO-donating atorvastatin NCX 6560 could show a better profile on skeletal muscle function and structure compared to atorvastatin.

Methods:
C57BL/6 mice received atorvastatin 40 mg/kg/day or an equivalent dose of NCX 6560 for 2 months. Muscle function was assessed treadmill test, serum creatine kinase (CK) activity, citrate synthase (CS) activity, and muscle histology.

Results:
Atorvastatin significantly (P<0.001) reduced muscle endurance, increased serum CK 6-fold, and induced muscle fiber atrophy. Conversely, NCX 6560 preserved muscle function, prevented CK increase and did not modify muscle structure. Interestingly, atorvastatin reduced CS activity, a marker for mitochondrial function, in gastrocnemius, diaphragm and heart, whereas NCX 6560 prevented such decrease.

Conclusion:
These findings suggest that NO may prevent statin-induced myopathy.

Abstract Abstrait

Aims Aim of the present study was to assess whether the addition of a nitric oxide (NO)-donating moiety to atorvastatin enhances anti-inflammatory and anti-atherogenic effects in an animal model of endothelial dysfunction, systemic peroxidation and inflammation, and accelerated atherosclerosis. But Buts de la présente étude était d'évaluer si l'ajout d'un oxyde nitrique (NO)-don radical à l'atorvastatine améliore les effets anti-inflammatoires et anti-athérogène dans un modèle animal de la dysfonction endothéliale, la peroxydation systémique et l'inflammation et l'athérosclérose accélérée.

Methods and Results Low-density-lipoprotein-receptor (LDLR) -/- mice kept on a high fat diet (HFD) for 16 weeks underwent photochemical-injury to the femoral artery with the local production of oxygen radicals. Méthodes et résultats de basse densité-lipoprotéine-récepteurs (LDLR) - / - souris soumis à un régime riche en graisses (HFD) pendant 16 semaines a subi des blessures photochimique-de l'artère fémorale à la production locale de radicaux oxygénés. HFD markedly enhanced cholesterol, inflammatory biomarkers in plasma and in the femoral arterial wall, and enhanced atherosclerotic lesions in the aortic arch; inflammation and atherosclerosis were further increased by photochemically-generated oxygen radicals. HFD cholestérol nettement améliorée, les biomarqueurs inflammatoires dans le plasma et dans la paroi artérielle fémorale, et l'amélioration des lésions d'athérosclérose dans la crosse aortique; l'inflammation et l'athérosclérose ont été encore accrue par les radicaux générés par photochimique.

Treatment with the NO-donating atorvastatin NCX 6560 (11.7 mg/kg), was significantly more effective than atorvastatin (10 mg/kg) in reducing: lipid-rich lesions in the aortic arch (surface covered: atorvastatin=24±5%; NCX 6560=14.7±3.9%, p<0.05), the production of radical oxygen species (ROS) in aortas (DCF fluorescence intensity/mg of protein: atorvastatin=2419±136.7; NCX 6560=1766±161.2, p<0.05), femoral artery intima/media thickness (atorvastatin=1.2±0.11; NCX 6560=0.3±0.14, p<0.05), circulating IL-6 (atorvastatin=34.3±6.8 pg/ml; NCX 6560=17.7±14.4 pg/ml, p<0.05) and matrix-metalloproteinase (MMP)-2 in the arterial wall (atorvastatin=55.2±1.9 ng/µg of proteins; NCX 6560=45.8±2.6 ng/µg of proteins, p<0.05). Le traitement avec le NCX donneur de NO atorvastatine 6560 (11,7 mg / kg), était significativement plus efficace que l'atorvastatine (10 mg / kg) dans la réduction: riches en lipides des lésions de la crosse aortique (surface couverte: atorvastatine = 24 ± 5%; NCX 6560 = 14,7 ± 3,9%, p <0,05), la production d'espèces radicalaires de l'oxygène (ROS) dans les aortes (DCF fluorescence d'intensité / mg de protéine: l'atorvastatine = 2419 ± 136,7; NCX 6560 = 1766 ± 161,2, p <0,05) , l'artère fémorale intima / média épaisseur (atorvastatine = 1,2 ± 0,11; NCX 6560 = 0,3 ± 0,14, p <0,05), IL-6 circulante (atorvastatine = 34,3 ± 6,8 pg / ml; NCX 6560 = 17,7 ± 14,4 pg / ml, p <0,05) et la matrice-métalloprotéinase (MMP) -2 dans la paroi artérielle (atorvastatine = 55,2 ± 1,9 ng / mg de protéines; NCX 6560 = 45,8 ± 2,6 ng / mg de protéines, p <0,05).

Conclusions In conditions of severe endothelial dysfunction, systemic peroxidation and inflammation and accelerated atherosclerosis atorvastatin, even at high doses, displays sub-optimal antiatherogenic and antiinflammatory effects while the addition of a NO-donating property confers enhanced and anti-atherogenic and antiinflammatory effects. Conclusions Dans les conditions de la dysfonction endothéliale sévère, peroxydation systémique et l'inflammation et l'atorvastatine athérosclérose accélérée, même à doses élevées, affiche sous-optimales effets antiathérogènes et anti-inflammatoire tandis que l'ajout d'une propriété donneur de NO confère effets améliorés et anti-athérogène et anti-inflammatoire.
